Waiver of Liability and Hold Harmless Agreement:

In consideration of being permitted to participate in any program or activity, including, without limitation, the Junior Experience at the 2023 U.S. Amateur Championship, offered by the City and County of Denver’s Department of Parks and Recreation – Golf Division and First Tee – Colorado Rocky Mountains, First Tee – Green Valley Ranch, First Tee – Southern Colorado, Gold Crown Foundation, Colorado Golf Association, Ball Corp., Boys and Girls Club, Cherry Hills Country Club, United States Golf Association (“USGA”), Colorado Uplift and Colorado PGA (herein collectively referred to as the “Releasees”),  I do hereby, for myself, my minor child(ren) and my heirs and administrators, waive and release any and all claims I may have against the Releasees, their employees and event contractors, various sponsoring agencies, and paid and non-paid instructors. Although I understand that a physician’s examination is not required for registration, it is highly advisable that I consult with a physician before participating in athletic or strenuous activities. I understand that if a participant’s behavior becomes disruptive or unsafe, he/she will be terminated from the program. I have read this form and understand its contents and request registration in this program offered by the Releasees.

I, (i) hereby release, waive, discharge and covenant not to sue the Releasees, their respective boards and their individual members, volunteers, or employees from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my minor child(ren) or me, or any of the property belonging to me, as result of, or in any way arising out of my child(ren) traveling to and or from any program or activity conducted by any Releasee in a vehicle or vehicles owned or operated by any Releasee; (ii) voluntarily assume full responsibility for any risks of loss; and (iii) hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed and enforced in accordance with the laws of the state of Colorado.

Photo release: 

I do hereby give the Releasees, their assigns, licensees, and legal representatives the irrevocable right to use my or my child’s name, picture, portrait, or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, which may be created in connection therewith.

This is the Privacy Policy governing the collection and use of information (i) provided by you through the websites located at  FirstTeeColoradoRockyMountains.org and any other websites, mobile applications, and other online and mobile services that link to this Privacy Policy  (collectively, the “Site”); (ii) gathered by First Tee through your use of the Site;  (iii) collected from you manually or in hard copy format (“Manual Services”), or (iv) provided to First Tee by a third party pursuant to your express consent for such information to be shared with First Tee.  When we refer to “your information” in this Privacy Policy, it refers to your personal information as well as your dependent’s personal information.

Please review this Privacy Policy carefully. If you do not want us to be able to gather and use information about you as set forth herein, please do not use this Site or Manual Services. This Privacy Policy identifies the nature of information that may be collected about or from you and the way in which such information may be used and disclosed.
